This Notice describes how medical information about you may be used and disclosed by OptymHealth and its affiliated licensed providers and pharmacies, and how you can get access to this information. Please review it carefully.

Uses and Disclosures of PHI

We use and disclose protected health information ("PHI") to provide treatment, obtain payment, and conduct routine health care operations. Examples include sharing your intake information with the prescribing provider, sending prescriptions to a licensed compounding or retail pharmacy, and arranging discreet shipping to your address.

Other Permitted Uses

We may also use or disclose your PHI as required by law, for public health activities, to report abuse or neglect, for health oversight, in judicial or administrative proceedings, to law enforcement when required, and to avert a serious threat to health or safety.

Your Rights

You have the right to inspect and copy your PHI, request amendments, request an accounting of disclosures, request restrictions on uses or disclosures, request confidential communications, and receive a paper copy of this Notice.

Authorizations

Most other uses or disclosures of PHI will be made only with your written authorization, which you may revoke at any time.

If you believe your privacy rights have been violated, contact us at hello@optymhealth.com. You may also file a complaint with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
